NMC and AHP registered professionals are encouraged to apply.

An exciting opportunity has arisen within Intermediate Tier Services, rated Outstanding by the CQC, for an Advanced Clinical Practitioner to join the multidisciplinary team at the Stamford Unit.

The Stamford Unit is a 96-bed Intermediate Care facility providing rehabilitation, reablement, complex discharge support, step-up admissions and end-of-life care. • Undertake comprehensive assessments, develop management plans, request and interpret investigations, prescribe where qualified and coordinate care to optimise patient outcomes.
  • Work collaboratively with GPs, Therapists, Nurses, Social Workers and specialist teams to deliver safe, effective and person-centred care.
