We are looking for a Transition Support Worker with excellent people skills to assist clients who are transitioning from an acute admission back into the community. This requires you to work in a targeted way to facilitate a smooth transition back into the community by providing cohesive, person-centred care that ensures continuity of support. This role targets patients with frequent admissions, aiming to minimise the risk of future readmissions through tailored, person-centred support. You will assist patients, offering both practical and emotional support to help them achieve greater independence, inclusion and choice. Your approach will be solution-focused, prioritising the specific needs and preferences of the individuals you support.

This is a unique role that offers exposure to skills development across services, business functions and coordination with inpatient and discharge teams. Responsibilities include liaising with community organisations, government agencies, families and carers, community services, housing departments, third sector organisations, and other relevant parties to ensure smooth transition to the community from hospital

Main duties of the job

Work with individuals assigned and prioritised by the Clinical Lead, providing tailored support and ensuring effective discharge planning and community integration.

Facilitate smooth transitions from acute care to community settings by assisting with discharge planning, prevention planning, and identifying triggers and coping mechanisms.

Link service users with relevant community groups and resources, promoting social inclusion and engagement. Provide recovery-focused support and assist with accessing opportunities both within the Trust and the wider community.

Build and maintain respectful and supportive relationships with service users, incorporating their own goals and support needs into recovery plans. Ensure interactions are sensitive to their ethnic, cultural, and social backgrounds.

Encourage and support service users in taking ownership of their care, promoting personal responsibility, self-awareness, and informed decision-making.
