Job Overview Deliver hospital-level care where it can make a real difference: in a patient's own home. RUH Hospital at Home (RUH@Home) is our consultant-led virtual ward, supporting patients who would otherwise need to be admitted to hospital, or who can safely return home earlier with the right clinical support around them. The service works closely with primary, community and social care partners to ensure safe transitions of care. You will join a multidisciplinary team of doctors, nurses, therapists, therapy assistants and healthcare assistants providing personalised acute care in patients' homes, seven days a week between 08:00 and 18:00. As a nurse within RUH@Home, you will have the opportunity to work with greater autonomy, assessing patients with acute and complex needs, developing individualised care plans, delivering evidence-based interventions and identifying and responding to changes in patients' conditions. You will work independently in the community, but you will never be working in isolation. You will be part of a wider multidisciplinary team and work closely with primary, community and social care colleagues to ensure patients receive safe, coordinated care. If you enjoy variety, autonomous working and building meaningful relationships with patients, while using your clinical skills to help people avoid an unnecessary hospital stay or return home sooner, this could be a fantastic next step in your nursing career.
