Main duties of the job

An exciting opportunity has arisen for the right candidate to join our Community Respiratory Service as Clinical Lead Respiratory Nurse Specialist based at Fartown Health Centre.

Our Community Respiratory Team delivers a community nursing service, Hospital at Home and pulmonary rehabilitation to individuals with chronic lung disease within the Greater Huddersfield area.

The team comprises experienced Specialist Nurses, Physiotherapists, Occupational Therapist, Exercise Physiologist and Health Care Assistants.

The successful candidate will have an extensive respiratory background and be passionate about leading high standards of care and management for patients diagnosed with chronic respiratory conditions.

You will provide clinical leadership to the nurses of our Locala Respiratory Specialist team to deliver tailored education for the development of self-management skills for patients and their carers'. You will also provide management for the avoidance of unplanned and unnecessary hospital admissions as well as using skills to provide specialist respiratory assessment of patients as part of Hospital at Home supporting hospital discharges. The role will include leading the development of services and delivery of high quality nursing interventions.
